The eCB system is also integral to regulation of emotional behavior, being essential to forms of synaptic plasticity that determine learning and response to emotionally salient, particularly highly aversive events [ 29 , 30 ]. Activation of CB 1 Rs produces anxiolytic effects in various models of unconditioned fear, relevant to multiple anxiety disorder symptom domains reviewed in [ 30 — 33 ].
Regarding conditioned fear, the effect of CB 1 R activation is complex: CB 1 R activation may enhance or reduce fear expression, depending on brain locus and the eCB ligand [ 34 ]; however, CB 1 R activation potently enhances fear extinction [ 35 ], and can prevent fear reconsolidation.
Genetic manipulations that impede CB 1 R activation are anxiogenic [ 35 ], and individuals with eCB system gene polymorphisms that reduce eCB tone—for example, FAAH gene polymorphisms—exhibit physiological, psychological, and neuroimaging features consistent with impaired fear regulation [ 36 ].
Reduction of AEA—CB 1 R signaling in the amygdala mediates the anxiogenic effects of corticotropin-releasing hormone [ 37 ], and CB 1 R activation is essential to negative feedback of the neuroendocrine stress response, and protects against the adverse effects of chronic stress [ 38 , 39 ].
Accordingly, CB 1 R activation has been suggested as a target for anxiolytic drug development [ 15 , 43 , 44 ]. In addition to dose-dependent activation of TRPV1 channels, the anxiogenic versus anxiolytic balance of CB 1 R agonists also depends on dynamic factors, including environmental stressors [ 33 , 49 ]. In preclinical studies, 5-HT 1A R agonists are anxiolytic in animal models of general anxiety [ 51 ], prevent the adverse effects of stress [ 52 ], and enhance fear extinction [ 53 ].
They are expressed on serotonergic neurons in the raphe, where they exert autoinhibitory function, and various other brain areas involved in fear and anxiety [ 54 , 55 ]. Mechanisms underlying the anxiolytic effects of 5-HT 1A R activation are complex, varying between both brain region, and pre- versus postsynaptic locus, and are not fully established [ 56 ].
Initial studies of CBD in these models showed conflicting results: When tested over a wide range of doses in further studies, the anxiolytic effects of CBD presented a bell-shaped dose—response curve, with anxiolytic effects observed at moderate but not higher doses [ 61 , 90 ]. All further studies of acute systemic CBD without prior stress showed anxiolytic effects or no effect [ 62 , 65 ], the latter study involving intracerebroventricular rather than the intraperitoneal route.
No anxiogenic effects of acute systemic CBD dosing in models of general anxiety have yet been reported. As yet, few studies have examined chronic dosing effects of CBD in models of generalized anxiety.
Anxiolytic effects in models used: Anxiolytic effects of CBD in models of generalized anxiety have been linked to specific receptor mechanisms and brain regions.
The midbrain dorsal periaqueductal gray DPAG is integral to anxiety, orchestrating autonomic and behavioral responses to threat [ 91 ], and DPAG stimulation in humans produces feelings of intense distress and dread [ 92 ]. The bed nucleus of the stria terminalis BNST serves as a principal output structure of the amygdaloid complex to coordinate sustained fear responses, relevant to anxiety [ 93 ].
In the prelimbic cortex, which drives expression of fear responses via connections with the amygdala [ 94 ], CBD had more complex effects: As noted, CBD has been found to have a bell-shaped response curve, with higher doses being ineffective.
Stress is an important contributor to anxiety disorders, and traumatic stress exposure is essential to the development of PTSD. In a chronic study, systemic CBD prevented increased anxiety produced by chronic unpredictable stress, in addition to increasing hippocampal AEA; these anxiolytic effects depended upon CB 1 R activation and hippocampal neurogenesis, as demonstrated by genetic ablation techniques [ 81 ]. Finally, CBD, partially via CB 1 Rs, decreased defensive immobility and explosive escape caused by bicuculline-induced neuronal activation in the superior colliculus [ 89 ].
Several studies assessed CBD using contextual fear conditioning. Briefly, this paradigm involves pairing a neutral context, the conditioned stimulus CS , with an aversive unconditioned stimulus US , a mild foot shock. After repeated pairings, the subject learns that the CS predicts the US, and subsequent CS presentation elicits freezing and other physiological responses. By contrast, CBD microinjection in the infralimbic cortex enhanced conditioned freezing [ 70 ].
Finally, El Batsh et al. In this study, CBD was administered prior to conditioning rather than prior to re-exposure as in acute studies, thus further directly comparable studies are required. CBD has also been shown to enhance extinction of contextually conditioned fear responses.
Extinction training involves repeated CS exposure in the absence of the US, leading to the formation of a new memory that inhibits fear responses and a decline in freezing over subsequent training sessions. Further studies showed CB 1 Rs in the infralimbic cortex may be involved in this effect [ 82 ]. CBD also blocked reconsolidation of aversive memories in rat [ 76 ].
Briefly, fear memories, when reactivated by re-exposure retrieval , enter into a labile state in which the memory trace may either be reconsolidated or extinguished [ 97 ], and this process may be pharmacologically modulated to achieve reconsolidation blockade or extinction. Overall, existing preclinical evidence strongly supports the potential of CBD as a treatment for anxiety disorders.
Activation of 5-HT 1A Rs appears to mediate anxiolytic and panicolytic effects, in addition to reducing conditioned fear expression, although CB 1 R activation may play a limited role.
While CBD predominantly has acute anxiolytic effects, some species discrepancies are apparent. In addition, effects may be contingent on prior stress and vary according to brain region.
Further receptor-specific studies may elucidate the receptor specific basis of this distinct dose response profile. Further studies are also required to establish the efficacy of CBD when administered in chronic dosing, as relatively few relevant studies exist, with mixed results, including both anxiolytic and anxiogenic outcomes. In particular, results show potential for the treatment of multiple PTSD symptom domains, including reducing arousal and avoidance, preventing the long-term adverse effects of stress, as well as enhancing the extinction and blocking the reconsolidation of persistent fear memories.
The anxiolytic effects of CBD in humans were first demonstrated in the context of reversing the anxiogenic effects of THC. CBD reduced THC-induced anxiety when administered simultaneously with this agent, but had no effect on baseline anxiety when administered alone [ 99 , ].
Further studies using higher doses supported a lack of anxiolytic effects at baseline [ , ]. By contrast, CBD potently reduces experimentally induced anxiety or fear.
CBD reduced anxiety associated with a simulated public speaking test in healthy subjects, and in subjects with SAD, showing a comparable efficacy to ipsapirone a 5-HT 1A R agonist or diazepam [ 98 , ].
CBD also reduced the presumed anticipatory anxiety associated with undergoing a single-photon emission computed tomography SPECT imaging procedure, in both healthy and SAD subjects [ , ]. Finally, CBD enhanced extinction of fear memories in healthy volunteers: These rCBF changes were not correlated with anxiolytic effects [ ]. In a series of placebo-controlled studies involving 15 healthy volunteers, Fusar-Poli et al. Response activation is diminished in PTSD and other anxiety disorders, and increased activation predicts response to treatment [ ].
CBD produced no changes in predicted areas relative to placebo but reduced activation in the left insula, superior temporal gyrus, and transverse temporal gyrus. The fearful faces task activates the amygdala, and other medial temporal areas involved in emotion processing, and heightened amygdala response activation has been reported in anxiety disorders, including GAD and PTSD [ , ]. CBD attenuated blood-oxygen-level dependent activation in the left amygdala, and the anterior and posterior cingulate cortex in response to intensely fearful faces, and also reduced amplitude in skin conductance fluctuation, which was highly correlated with amygdala activation [ ].
Dynamic causal modeling analysis in this data set further showed CBD reduced forward functional connectivity between the amygdala and anterior cingulate cortex [ ]. Epidemiological studies of various neuropsychiatric disorders indicate that a higher CBD content in chronically consumed cannabis may protect against adverse effects of THC, including psychotic symptoms, drug cravings, memory loss, and hippocampal gray matter loss [ — ] reviewed in [ ].
As THC acutely induces anxiety, this pattern may also be evident for chronic anxiety symptoms. Two studies were identified, including an uncontrolled retrospective study in civilian patients with PTSD patients [ ], and a case study in a patient with severe sexual abuse-related PTSD [ ], which showed that chronic cannabis use significantly reduces PTSD symptoms; however, these studies did not include data on the THC: Thus, overall, no outcome data are currently available regarding the chronic effects of CBD in the treatment of anxiety symptoms, nor do any data exist regarding the potential protective effects of CBD on anxiety potentially induced by chronic THC use.
Evidence from human studies strongly supports the potential for CBD as a treatment for anxiety disorders: Limited results in healthy subjects also support the efficacy of CBD in acutely enhancing fear extinction, suggesting potential for the treatment of PTSD, or for enhancing cognitive behavioral therapy.
Further studies are also required to establish whether chronic, in addition to acute CBD dosing is anxiolytic in human. Human experimental findings support preclinical findings, and also suggest a lack of anxiogenic effects, minimal sedative effects, and an excellent safety profile. Overall, this review emphasizes the potential value and need for further study of CBD in the treatment of anxiety disorders. Out of interest, I started my research because I bought some CBD paste to try for my crippling arthritis pain. It was horribly expensive and came from a guy, who knew a guy, who said it was really Endoca, but had no labels. It really worked so well, I stopped Mobic and was walking without a brace. The shady guys had been shut down when I went to reorder. I also discovered that Endoca is not available in Australia where I live.
Hence my research into something hopefully as good who knows what I had and even more hopefully, at an affordable price. Now I have spent so much money experimenting, I could have bought a return ticket to personally visit Mr. Endoca and buy up half his stock. Please, please Consumer Lab can you get on to testing for us? I know I am not alone in my frustration at how hard it is to know what's what in this industry. I use the rubs for severe spinal stenosis and arthritis.
My doctor says Young Liiving's Copaiba oil has a higher concentration of pain relief. Is there a difference between "dietary supplements" and "supplements" - this distinction appears to be made here in the review as to legality?
Or is this reference to legality only referring to the difference between industrial hemp derived CBD and the medical marijuana controlled plant?
I've noticed an explosion of CBD products, hemp oils, etc. Could you clarify in a more obvious way these subtle points. Medical use of CBD is legal in many, but not all, states, as noted above, but the products cannot necessarily be legally sold in those same states. Hope that helps a bit. It is tricky and evolving area.
It is still not clear to me. Tricky is an understatement. It appears that it was the DEA that put it on schedule 1 in Dec. DEA" legal case 14 years ago, it is being challenged again? What I don't understand is how it can illegal, yet sold nearly everywhere, at the same time? It is illegal and legal at the same time? A kind, generous friend gave my wife a bottle to help with her RA symptoms while we were vacationing in Canada.
She will not use it until we find out if it is legal to possess and her doctors give her the go ahead, but we would be interested in the test results. I have been a Consumerlab member for years. We will consider that. We have received many suggestions this week of products.
He has arthritis and a bad heart murmur. At first it really seemed to help with his pain and he was much more mobile. After a few weeks he started getting more lethargic and not eating. I did some research and discovered it should not be given along with any medications that are metabolized by the liver. It causes these meds to build up in the body so he was exhibiting symptoms of overdose of his heart medications. I stopped the CBD oil and he got started eating and seemed to feel better.
Give with caution if your pet is on other meds. You can read the manufacturer's claims. I'm 87 and found the CBD oil does relieve my aches and pains, and clears my mind. The results aren't like prescription drugs; they aren't usually immediate. People have differing results. There are recommendations for the amount to take and you are wise to heed them. I use Blubird Botanicals and ran out a couple days ago, and I can definitely feel the difference.
I expect a new order tomorrow. For me, the expense is worth it. There are several online sites where you can receive much information. Beware of the Pure Natural brand. It offers a free trial, but in the small print you are signing up for a monthly supply at full cost. The BBB has a report on this. I meant to say that CBD oil isn't made from the seeds, from what I've read.

In Az medical marijuana is legal with a card from a physician. I didn't see any relief with it. I get instant relief for anywhere between a half and hour to a couple of hours.
My pain is reduced substantially while waiting for the pain pill to take effect. I'm NOT recommending anything here, just letting you know how it helps me. By the way, I'm 71 and have been with this pain for over 5 years since the surgery. It is some help.

Instead, it uses carbon dioxide to extract the full range of chemical components from the flower buds and then distill them into dense CBD oil.

The upsides of lipid-based extraction are that the fat helps make the CBD more bioavailable easy to absorb , and there are no harsh solvents used. Condensed CBD oil can be taken as a thick paste, but this is the least pleasant option. More commonly, the CBD oil is mixed with a carrier oil, such as hemp oil or coconut oil, to a specific concentration of CBD.
The distinctive taste — which comes from the terpenes and not the cannabinoids — is often masked with chocolate, mint, or other flavorings. It typically comes in a small bottle with a dropper to administer the oil mixture. The best way to take CBD oil mixed with a carrier oil to a specific concentration is to place a few drops or dropperfuls under your tongue for 15 seconds to access the sublingual gland.
There, the CBD is absorbed directly into the bloodstream called sublingual consumption or administration for the fastest acting effects. Another method is to take a few drops or dropperfuls orally, swish the liquid around in your mouth, and then swallow it. CBD oil mixed with a carrier oil can also be taken as soft-gel capsules to avoid any taste, but absorption is only through the intestinal tract.
The average dose range is mg of CBD, one to three times per day, though much higher doses of mg sometimes required to control pain are equally well tolerated. Some people will notice benefit at the lower end of the dose range, but most people will need mg to notice any effects.
Because different products provide different concentrations of CBD, the packaging usually states how much CBD is in the entire bottle as opposed to the amount in a certain number of drops or dropperfuls, so measuring can be a little tricky. A dropperful of the medium grade product mg of CBD per fluid ounce will deliver about 15 mg of CBD — a good starting dose.
And a dropperful of a high concentration product mg CBD per fluid ounce will provide about 50 mg of CBD per dropperful. CBD oil is also available as soft gel capsules. With these, the mg quantity of CBD should be designated per capsule. Because some of the chemical compounds in capsules are lost during digestion, you may find you need to take a little more to experience the benefits.
As with any medicinal herb, start at a low dose and gradually build up to a higher dose as you get used to the effects of the substance. Most people notice benefits almost immediately, but some experts suggest that full benefit does not occur until after a couple of weeks of consecutive use.
CBD isolate which is CBD alone acts very differently in the body than a spectrum of hemp chemical components. Here are some quick definitions:. The cannabis plant naturally generates cannabinoids, terpenes, and other chemical compounds to serve different functions in the plant. These functions include regulatory properties, potent antioxidants, and protection from microbes and insects.

When CBD is formally legalized at the national level, prescription drugs providing high doses of purified CBD will become available several are already in the pipeline. Reported side effects of hemp oil with CBD are generally mild and uncommon and can include tiredness, loose stools, and mild changes in appetite and weight either increased or decreased.
Both hemp oil with CBD hemp flower-bud extracts and purified CBD CBD isolate have been shown in both animal and human clinical trials to be remarkably safe and well tolerated. Prolonged use is not associated with an increased risk of side effects. In research studies , up to mg of purified CBD per day has been used to address various medical illnesses without reported harmful effects including changes in heart rate, blood pressure, temperature, oxygen and carbon dioxide levels, electrolyte balance, gastrointestinal function, psychomotor functions, or sleep cycles.
Prolonged use at high doses has not shown potential for abuse of CBD. In fact, a clinical study published in found that recreational polydrug users did not show abuse potential with use of CBD. Long-term studies have not evaluated potential changes in hormonal balance or long-term adverse changes in liver function, though prolonged use of CBD enhances metabolism of certain drugs. Stopping CBD oil suddenly has not been associated with withdrawal effects.
While the cost of CBD oil products is presently high, it will likely come down dramatically after CBD and hemp are legal by federal standards — but prices will still vary widely. Reputable companies selling CBD oil products will state the CBD concentration and extraction methods on the bottle or website. Typically, the concentration is stated as milligrams mg of CBD per fluid ounce though some products standardize mg of CBD to milliliters ml.
The benefit comes from the amount of CBD consumed, not the amount of oil. To calculate the cost per milligram of CBD, simply divide the dollar amount of the product by the total milligrams of CBD in the bottle. As for extraction methods, remember that vapor distillation and CO2 extraction are preferred.
Lipid-based extractions will likely fall in the middle price-wise. The highest quality cannabis is grown indoors , so quality standards can be controlled. Use of clean water and organic methods of farming that are free of pesticide use and unnatural fertilizers are, of course, preferred. With outdoor-farmed cannabis, quality standards and potency are not as easily controlled. Taste can be a sign of value, too. Poor quality oils will have a very unpleasant chemical taste, and they can cause significant burning to mouth tissues.
A good quality product should be smooth and not cause significant burning. Interestingly, the best quality products are associated with a distinct cannabis taste, indicating that the full spectrum of chemical components with trace levels of THC are present.
Even in high doses, CBD oil will not cause euphoria or impair coordination, balance, or motor functions. Use of CBD oil is associated with improved sense of well-being, but not an exaggerated feeling of well-being.
Use of CBD oil has never been associated with hallucinations or abnormal mental activity. Creams and salves for musculoskeletal discomfort generally contain very small amounts of CBD that are absorbed through the skin. Many of these products do provide significant benefit, however, but the benefit is likely derived from other aspects of CBD — especially terpenes from cannabis and essential oils, thanks to their anti-inflammatory properties.
Though CBD will likely become a highly promoted ingredient in beauty products, it is unclear whether the concentration of CBD present in these products will provide a benefit. There is near zero potential of becoming habituated to hemp CBD products. It should be noted that recreational use of marijuana high THC, low CBD cannabis does result in dependence but different from narcotics or alcohol, and not as debilitating.
Chronic use of THC may be associated with atrophy in certain areas of the brain and reduction of certain cognitive functions at this point, studies are not conclusive. Interestingly, studies have shown that taking CBD oil regularly can restore areas of the brain that have become atrophied in marijuana abusers.
There have been no reports of anyone overdosing on cannabis. This sets even recreational use of cannabis widely apart from narcotics and alcohol, both of which can cause severe respiratory depression and death at excessive doses.
