Slianklin In a thorough search of the literature the author failed to find a single paper describing histological changes in the human hypophysis following burns; more- over Baer does not include a single reference to the pathology of the hypophysis in his Annotated Bibliography on Pathology and Physiology of Burns.
It is the purpose of this paper to describe the histology of the hypophyses from six patients with burns taken at autopsy at the Medical College of Vir- ginia. METHODS Six out of hypophyses collected during routine examination from autopsies durkig a twelve month period were from burn cases.
One half of each hypophysis was fixed in Bouins and stained by the hematoxylin-eosin and the Masson trichrome method; the other half was fixed in formalin-ammonium-bromide and impregnated by the Hortega silver carbonate method. Case 1, Autopsy No. An eight-year old girl who sustained second and third degree burns over 47 Vo of the body and died 47 days later from sudden unexpected cardiac arrest. The pathologist found nothing that could not be attributed to the burn. The pars anterior can, on Ihe basis of differences in colouring in Masson stained sections, be divided into four zones; central Fig.
Some pyknotic nuclei arc present, usually in the acidophils. On passing peripherally, changes take place in the staining of the cytoplasm and the granules. Early evidence of changes is the appearance of a small amount of light blue cytoplasm in the acidophils. This increases in amount and intensity on passing into the transitional zone. The granules ' of the acidophils also change from bright to dull brick red.
In the same area there is an increase in the cytoplasm of the chromophobes and they begin to accumulate some acidophilic granules. The irnnsHionul zone Tig. The large ones are acidophiles and the smaller atypical chromophobes. Basophils cannot be identified. The next zone Fig. Both the cytoplasm and granules of the cells are stained lavender. There is a marked increase in the cytoplasmic volume, loss of cell membranes and fused cyto- plasmic masses.
By following transitional stages from the central, through the tran- sitional, into the lavender coloured zone, it can be stated that cells in this area are derived from the acidophils and atypical chromophobes. There is no increase in the number of pyknotic nuclei on passing from the central to the transitional zone; how- ever, there is a fairly sharp increase in the lavender stained zone.
The peripheral zone is featured by extensive degeneration Fig. A high percentage of the nuclei show varying degrees of pyknosis and the cytoplasm is fragmented, sharply reduced in amount and often vacuolated. In spite of the changes observed, vacuoles arc only present in the most peripheral zone of degenerating cells. No marked changes are noted in the connective tissue or the blood vessels.
Even in 'the lavender stained zone the collagenous fibers arc stained blue. Pars nervosa appears to be normal. There arc no PAS positive cells basophils in the proximal third nor in the dark lavender zone; in the remaining part there are scattered areas of well stained PAS positive cells. The total number of basophils is far below the average. Case 2, Aitlopsy No. The pathologist reported the following findings; Edema of the brain, a circumscribed tumour nodule of 1 cm.
The central portion of the proximal half of the pars anterior has a large area o hypcrchromatically stained acidophils. On passing peripherally from this area the acidophils become progressively darker stained and change from bright red to reddish- purple.
Very few basophils arc seen in the proximal half and except for well stame colloid and reticulum material in the area it is PAS negative. In the distal part there arc scattered acidophils, many of them stained atypically, and some basophils, basophils are concentrated anteriorly and medially and are strongly PAS positive.
All photographs arc from the different parts of the same section. Through the central area. Through the transitional zone. Through the lavender zone. Through the peripheral degenerating zone. The cytoplasm and granules be come stained dark lavender as described in Case 1. Some fairly typical basophils are present in the central area. In places the large vacuoles give the area a honeycomb appearance. The paucity of PAS stained cells lends support to the finding with the Masson stained sections that this hypophysis contains very few basophils.
The pars nervosa is normal and the fairly large colloid filled follicles in the pars intermedia are well within normal limits.
Case 6, Autopsy No. A year old male with third degree burns of both legs covering 29 Vo of the total body surface. He died three days after the accident. The pathologist found an abscess of the lung with ml. The hypophysis shows marked fibrosis of the pars anterior and the epithelium of the pars intermedia has been partially replaced by broad bands of collagenous fibers which extend into the proximal part of the pars nervosa.
Among them in the pars nervosa are numerous almost completely degenerated basophils. Numerous colloid filled follicles are present in all parts of the pars anterior.
The anterior lobe can be divided into three major areas. A relatively small central area containing mostly acidophils, a poorly defined light lavender zone and a broad outer zone.
In the proximal area there are many acidophils, also chromophobes and some baso- phils. Many of them show various degenerative changes. In the lavender zone, cells are undergoing changes similar to those described in Case 1.
Beyond this zone the fibrosis is more extensive and nearly all the cells are in various stages of advanced degeneration. Extensive vacuolization of the cells, as in Case 5, is not found in this hypophysis. In spite of the extensive degeneration there remain a considerable number of baso- phils as shown by PAS positive cells. They are located especially in the central part and spread through the distal part of the pars anterior. The hypophyses of four dogs numbers 7, 9.
In the Masson stained sections the pars anterior of all four hypophyses can be divided into a large central portion and a thin peripheral zone; the larger central area is stained a bright red, the outer zone a much lighter red. The central part is filled with hyper- chromatically stained acidophils and chromophobes with scanty cytoplasm having some light reddish granules.
In some places a number of nuclei, which appear to have no cytoplasm, are closely packed together.
